Referring Physician

Easy steps to make a referral:

1. Download and fill out the editable Physician Referral Form
2. Save the form to your computer
3. Use the below online form to upload the edited referral form to a specialist. Or, you may print and fax the referral form to (601) 932-4845

After the form is submitted, the patient will be contacted by a NS2 specialist to complete the medical intake and to schedule an appointment at the appropriate clinic.
